Supporting vulnerable people – A Smart template suite for use with patients who are refugees, asylum seekers, homeless and at risk of homelessness

As a result of war, climate change, food shortages and disease outbreak the world is experiencing staggeringly high levels of migration. On top of this the cost-of-living crisis is driving more people into poverty domestically, creating a cohort of vulnerable people with specific health needs. To ensure we can effectively meet these health needs the Clinical Effectiveness Group (CEG) collaborating with North-East London Integrated Care Board (ICB) have developed a suite of clinical templates, searches, protocols and dashboards. We have currently identified refugees, asylum seekers, homeless and at risk of homelessness as our vulnerable people cohort. The templates are able to adapt to the specific health needs of the patient you are seeing to include as appropriate, mental health and well-being, vaccinations, safeguarding, language needs as well as other routine health care needs. The templates will also include signposting, documents and referral options. We will demonstrate how we use these tools locally and discuss the issues identified and resolved as part of our local services across the region. We will demonstrate how these tools have helped east London practices achieve year on year improvement in meeting these important targets.
